Babak Azizzadeh MD
General Partner

Dr. Azizzadeh is a Facial Plastic and Reconstructive Surgeon, board-certified by the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology-Head and Neck Surgery. He is one of the owners of the La Peer Health Systems, a medical organization specializing in various fields including Plastic & Reconstructive Surgery. He is also the founder and president of the FPBPF (Facial Paralysis & Bells Palsy Foundation), a non-profit organization committed to the treatment of individuals with facial paralysis and Bell’s palsy. The U.S. News & World Report recently awarded Dr. Azizzadeh with the prestigious ranking of “Top Doctor” in his medical field.

Dr. Azizzadeh is co-director of the facial plastic and reconstructive surgery fellowship at the Keck School of Medicine at USC and co-chairman of the Cedars-Sinai Annual Advances in Multispecialty Aesthetic and Reconstructive Surgery Symposium. Besides his role in FPBPF, Dr. Azizzadeh is involved with several other non-profit charity organizations such as the Global Smile Foundation, Operation of Hope, Face to Face and the R.O.S.E Fund. Dr. Azizzadeh is among the handful of surgeons in the US, who perform repair of facial paralysis.

Dr. Azizzadeh undertook his medical education at the David Geffen School of Medicine at UCLA. He did his surgical training at the Harvard Medical School, Cedars-Sinai Medical Center, and David Geffen School of Medicine at UCLA.

Babak Azizzadeh performed the corrective surgery for Mary Jo Buttafuoco, who was shot in her face. The surgery was a very successful one and was documented by media all across the United States. He appeared on The Oprah Winfrey Show during May 2005, explaining corrective surgery for facial paralysis to the viewers along with his patient. The same year, he also appeared on the Entertainment Tonight with the same patient. His work in Zimbabwe was covered by the People magazine. His Cosmetic Cocktail treatment regimen was also covered by the US media. In 2009, Azizzadeh appeared on the Discovery Health Channel explaining revision rhinoplasty.

Ravin Bisla is the founder of DealOctane, a corporate finance and M&A advisory firm focused on early-stage growth companies. Prior to DealOctane, Ravin served as a Transaction Director for DaVita Healthcare Partners for over three years where he closed dozens of transactions totaling over $200 million in total value. Prior to DaVita, Ravin spent five years with Vantage Oncology in various business development capacities including acquisitions, physician recruiting and growth initiatives – driving over $75 million in transaction value. Ravin received his MBA from UCLA Anderson and his undergraduate degree in economics from Duke University.

Chris Bright
General Partner

Chris Bright is the Chief Operations Officer of Integrated Oncology Network, LLC. Prior to ION, Mr. Bright was the Chief Operations Officer at SightLine Health and assisted in the startup of SightLine in July 2005. SightLine Health is a leading, national healthcare company that builds and operates patient-focused facilities providing advanced, non-invasive treatment options for cancer. He is the former Divisional CEO of SightLine Development Company and was responsible for joint ventures, strategic alliances and partner relations. Previously, he was the National Sales Director for Ramic Medical Imaging, which consisted of thirty-eight multi-modality outpatient diagnostic imaging centers.

TJ Farnsworth

TJ Farnsworth has a twenty-year history of innovation and successful start-ups. In 2005, TJ was the founder of SightLine Health, which grew to one of the largest providers of radiation oncology services in the US, with clinical operations in Texas, California, Washington, Kansas, and Colorado. SightLine Health was listed on the Inc. 500 list of fastest growing private companies in the US for five consecutive years, and in 2010 was named the fastest growing company in Houston by the Houston Business Journal and the fourth fastest growing healthcare company is the US by Inc. Magazine.

After a 2-year journey of infertility, TJ resigned his position as the CEO of SightLine Health on a mission to ensure that patients coming after he and his wife would not have many of the same frustrations they experienced. TJ maintains his position on the board of directors of SightLine Health and remains engaged in the continued success of his first company. TJ also sits on the board of directors of Mammoplan, the developer of a women’s health app, and CureDuchenne, a non-profit dedicated to finding a cure for Duchenne muscular dystrophy. TJ holds a BA in Government and a BS in Economics from the University of Texas at Austin. He was named one of Houston’s 40 under 40 in 2011 and he was the 2014 winner of the Ernst & Young Entrepreneur of the Year for Healthcare.

Jeffrey Goffman
General Partner

Jeffrey Goffman is a general partner of HC Angels. Mr. Goffman is the Chairman, CEO and Co-Founder of Integrated Oncology Network, LLC (“ION”). ION is a national oncology management company that partners with hospitals, medical systems and physicians. ION has partnerships in 21 cancer centers with more than 250 physicians, and several prestigious hospital providers including HCA, Providence / Covenant, Cedars Sinai, IASIS, Orlando Health and others. Previously, Mr. Goffman was the CEO and Founder of OnCURE Medical Corp. (“OnCURE”). OnCURE was a private equity backed and one of the largest providers of outpatient radiation oncology care in the United States until sold in 2006. Mr. Goffman was also a co-founder of Affiliated Pet Systems, LLC, a private company that provides PETCT services in the Maryland / Washington D.C. area, which was sold in 2015. Mr. Goffman was the CEO and Founder of U.S. Diagnostic, Inc. (“USD”), a leading consolidator of outpatient diagnostic imaging facilities. Mr. Goffman took USD Public in 1994.

Mr. Goffman was previously a board member of private equity owned Solis Women’s Health, which was sold in 2015. Mr. Goffman is currently a board member of Appian360, LLC and Team Better, LLC.

Mr. Goffman is an inactive Certified Public Accountant licensed in New York and Florida and holds a Bachelor of Science degree in Accounting from the Barney School of Business at the University of Hartford. Mr. Goffman is the Chair of the American Cancer Society’s CEO’s Against Cancer California Chapter, and serves on the non-profit boards of CureDuchenne and Wings for Crossover.

Grant Ingersoll
General Partner

Grant Ingersoll is the President and Co-Founder of Integrated Oncology Network, LLC. Prior to ION, Mr. Ingersoll was the Director of Corporate Development at Vantage Oncology and OnCURE Medical Corp., two of the largest radiation oncology providers in the US. Grant has also worked at Microsoft, in investment banking at UBS and as a management consultant at Wipro working with Fortune 500 companies around the globe. Grant received his MBA from the University of Southern California and his undergraduate degree in finance from Brigham Young University.

Kamran Jamshidinia DPM
General Partner

Dr. Jamshidinia is a board certified Foot & Ankle Surgeon.  Dr Jamshidinia is Board Certified by American Board of Foot & Ankle Surgery and is a Fellow of the American College of Foot and Ankle Surgeons. Dr. Jamshidinia received his Bachelor of Arts in Biology at California State University Northridge before attending California College of Podiatric Medicine, San Francisco where he received a Doctor of Podiatric Medicine Degree.

His post-doctoral training was thru a combined residency program thru the Department of Veterans Affairs and Olive View UCLA, where Dr Jamshidinia completed a two year residency with training in all aspects of elective foot surgery, reconstructive foot surgery, ankle trauma/ORIF, diabetic limb salvage and amputation for the emergent limb threatening foot infection and complex wound care and inpatient management.

Dr. Jamshidinia has affiliations with the Department of Veterans Affairs Hospital, West Los Angeles, CA, Cedars Sinai Medical Center where he has been an attending for the past 16 years.

Dr. Jamshidinia’s teaching posts include Department of Veterans Affairs West Los Angeles, 2001 to present and Cedars Sinai where he participates in the residency training programs.  Dr Jamshidinia also serves as the foot and ankle specialist for the California Rehab Institute which is a joint venture of Cedars Sinai Medical Center and UCLA.  Here Dr. Jamshidinia is a consulting physician providing Podiatric care for patients after long complex hospitalizations.

Babak Larian MD
General Partner

Dr. Babak Larian is the Clinical Chief of the Division of Otolaryngology, Head and Neck Surgery at Cedars-Sinai Medical Center. He is a board certified Diplomate of the American Board of Otolaryngology/Head and Neck Surgery. He is a Clinical Instructor of Surgery at the David Geffen School of Medicine at UCLA. He has written numerous peer-reviewed articles, and has presented his findings in both national and international conferences.

Dr. Larian received his bachelor’s degree from University of California, Irvine and his medical degree from UCI as well. He then completed his internship in General Surgery and residency in Otolaryngology/Head and Neck Surgery at UCLA.

Dr. Larian is involved in a variety of charity organizations including Medical Missions for Children and Global Smile Foundation. He is the Medical Director of the Los Angeles chapter of HELPS International Medical Mission, organizing and leading a team of 100 volunteers in a MASH type medical/surgical operation serving rural Guatemala since 2003. He is also on the board of International Society for Children with Cancer.

Dr. Larian has over 12 years of extensive management experience as part of Streamline Management, LLC, a company that has successfully managed and expanded LaPeer Surgery Center, and PathMD pathology laboratory. In the process helping to recruit over 100 physicians into these 2 businesses, as equity shareholders. He continues to explore entrepreneurial possibilities, specifically as it relates to fields of medicine.

Kia Michel MD
General Partner

Dr. Kia Michel is a highly respected urologist in Los Angeles with expertise in robotic and minimally invasive surgeries. He is the founding member of Comprehensive Urology Medical Group, located at the Cedars-Sinai Medical Building. He is currently the Director of the Prostate Cancer Institute of Los Angeles, an organization dedicated to providing state-of-the-art care for men with prostate cancer, including focal prostate cancer therapy with HIFU.

Dr. Michel has also co-founded and led several medically related businesses. He is the co-founder and managing member of Lapeer Surgery Center, a multi specialty center in Beverly Hills. Dr. Michel is also the co-founder and managing member of PathMD, a regional multi specialty anatomic pathology laboratory. Dr. Michel has also started two investment entities: BluSky Investment Group and BluFrog Path Lab Solutions that are involved in investing and funding medically related business ventures.

Dr. Michel has an illustrious academic career. Dr. Michel graduated from Whitman College where he was a presidential scholar. He subsequently attended the University of Washington School of Medicine where he graduated with honors. After completing his medical studies, Dr. Michel completed both his residency in Urologic Surgery and a fellowship in Urologic Oncology at UCLA, where he was recognized as a Pfizer Scholar. Dr. Michel did extensive cancer research while at UCLA and his efforts were recognized by the National Cancer Institute, the National Institute of Health, and the American Urological Association.

Siamak Tabib MD
General Partner

Dr. Siamak Tabib is a board certified gastroenterologist and hepatologist actively practicing in the Beverly Hills/West Los Angeles area. He is the recipient of the Best Doctor award by SuperDoctors.com for the last six consecutive years. He has also repeatedly won the Vitals Patient’s Choice Award and Vitals On-time Doctor Award. His expertise has been showcased on CBS News, ABC News, Fox News, Beverly Hills-213 Magazine, US World & News, and Voice-of-America, the official external broadcast institution of the United States federal government.

Dr. Tabib is the co-founder and managing member of multiple healthcare and real estate entities including La Peer Health Systems, BluFrog Path Lab Solutions, BluFrog Pathology Holdings, PathMD, BluFrog REI, Streamline Management, and Specialized University Pathologists. He is also the co-founder and managing member of BluSky Investment Group, LLC, an investment arm active in investing and managing healthcare and human resources entities.

Dr. Tabib received his bachelor’s degree from UCLA and his medical degree from the David Geffen School of Medicine at UCLA. He then completed his internship and residency in Internal Medicine at Cedars-Sinai Medical Center. Following this training, he completed his fellowship in Gastroenterology and Hepatology at the combined UCLA/Cedars-Sinai medical program. He currently serves as the Assistant Clinical Professor of Medicine at UCLA. He has authored countless peer reviewed articles and textbook chapters and has an extensive research background in the field of digestive diseases. He has presented and has been a panelist both nationally and internationally in medical and healthcare related subjects including healthcare policy.
